Online Substance Abuse Assessments / Evaluations

If enrolling for an Evaluation DO NOT enroll in any other classes until your evaluation is completed and a recommendation is made.

Online Substance Abue Evaluations are for those required to complete an evaluation for a DUI, alcohol or substance offense. The purpose of an evaluation is to evaluate alcohol or substance problems that may require education or treatment.

Online DUI Evaluation Requirements: You will need access to the internet and a video camera device on your phone, tablet or computer. We will require discovery documents from your attorney containing your arrest record. We will require written authorization to release your report to others.

Additional reports, forms or documentation fees are $25 - $50 based on the complexity of forms or reports needed.

Why do I need an evaluation?
Most states require persons arrested for driving under the influence of alcohol and/or other drugs (DUI) to undergo an alcohol and drug evaluation before driving privileges can be granted by the Court.

What does the evaluation consist of?
The purpose of the evaluation is to determine the extent of the motorist's alcohol and/or drug use and risk to current or future public safety. The following are required for an evaluation: Review of Driving history, chemical test results (BAC), and Objective Tests for Alcohol and Drug use.
